How to make an appointment
FOR GENERAL PRACTITIONERS
- Kindly submit the details of your patient using the online form below. Our receptionist will then contact your patient to arrange an appointment as soon as possible.
- Alternatively, you can fax your referral to our practice on 9312 2615 and we will contact your patient for an appointment.
- If you wish to refer your patient to see one of our allied health professionals, it would be useful to consider organising a Team Care Arrangement.
